SUMMARY:

Health is guaranteed as a human right and is therefore an instrument of transformation aimed at social justice related to full citizenship and equity. However, the dynamics of stigma and discrimination against lesbianand bisexual women manifest themselves in symbolic barriers that prevent access to health actions and services, and even their free movement across their territory. In this sense, this study aims to understand the reality of integrated health care for lesbian and bisexual women in the West Zone of Rio de Janeiro. It is an intervention study based on a mainly Latin American theoretical and methodological framework, combined with interviews analyzed through intersectionality with the aforementioned population. The research revealed that inside formal healthcare spaces in the territory, there is no protocol for the sexual healthcare of lesbians and bisexuals, and points to a lack of specific knowledge about the impacts of social determination on the health-care-illness process through stigmatizing and discriminatory social dynamics that are LGBTphobic, sexist, racist, and classist, perpetuating a place of marginalization in formal healthcare settings. Fieldwork within four collectives and social organizations in Rio de Janeiro's West Zone highlights the power of community connection and the construction of comprehensive healthcare networks, anchored in ideals of good living and collectivity. Interviews with lesbian or bisexual women in the territory, aged between 20 and 36, who are not organized in collectives reveal that lesbophobia and biphobia, which are morally and culturally naturalized in the West Zone, can prevent these women from moving safely around their own territory, which can lead to chronic stress and possible health problems. This population is left with the almost unilateral responsibility of devising personal strategies for health care and even safety. Groups of women or groups focused on promoting and valuing cultura in the West Zone absorb some of these issues, and when there are no support networks, women leave their own territory in search of a fuller and healthier life. The research points to the existence of a border identity among these women, assimilating the unique construction of their identity in that territory. Thus, it points to present and future possibilities for coordination between community agents and existing care networks in the West Zone, in order to absorb the needs of the lesbian and bisexual population and include them in the daily routine of formal health spaces to realize their right to health in an equitable manner.
