Adolescence is pointed out in literature as a period of physical, biological, emotional and social changes. The evaluation capacities are less evolved and are expressed, often, in impulsive acts and search for new sensations. Impulsive behaviors co-occur with neural, structural, and functional developments in the adolescent's brain, in particular, with inhibitory control, a cognitive ability related to a wide cortico-subcortical network that matures only after adolescence. The ontogenic changes affecting the neural circuits involved in inhibitory control and reward-related processes are relevant to adolescent risk behavior. In this sense, this systematic review of the literature discusses the results of 51 articles, selected from inclusion and exclusion criteria, from the databases Scielo, PsycInfo, PubMed and Lilacs on the theme of inhibitory control, impulsivity and alcohol and drug use for adolescents. The results, in general, showed that adolescents with greater involvement in substances tend to present higher levels of impulsivity than those with less involvement. Those who were exposed to alcohol in the prenatal period showed immature and inefficient neural patterns. In addition, neurocognitive changes that were associated with impaired decision-making (impulsivity, weak inhibitory control, emotional changes) and self-regulation were related to changes in the functioning of the prefrontal cortex. The effects of substances on brain structure and function were mainly seen through the differentiated activation of the areas of pre-frontal and oculomotor control, brain regions that were associated with inhibition of the effective response. Understanding predictive factors helps identify at-risk youth for prevention efforts, as well as create interventions targeting cognitive impairments or brain regions involved in substance use.
