SUMMARY:

Chronic kidney disease leads to bodily changes, disruptions in daily activities and interpersonal relationships, affecting the identity and emotional state of individuals diagnosed with this condition. This situation can compromise patients’ engagement in their own care process, including the acceptance and adherence to therapeutic guidance and prescriptions. This qualitative and descriptive study aimed to investigate the perceptions of chronic kidney disease patients, their family members, and healthcare professionals regarding the factors and strategies that influence adherence and facilitate engagement in hemodialysis treatment. Semi-structured interviews were conducted with 12 professionals from different specialties working at the hemodialysis clinic of a university hospital located in a city in Rio de Janeiro’s state, as well as with five patients under their care and their accompanying family members. To guide data collection, interview scripts containing open-ended questions were developed to explore, from the participants’ perspectives, the challenges and barriers to adherence to hemodialysis treatment, as well as professional actions that could facilitate patients’ adherence and engagement in this process. Data were analyzed using Bardin’s Content Analysis. Among patients, the Brazilian validated version of the End-Stage Renal Disease Adherence Questionnaire (ESRD-AQ) was also applied. Difficulties in controlling fluid and dietary intake, lack of knowledge about treatment, and physical, social, and work-related changes were identified as barriers to adherence and engagement. Conversely, family presence, care provided by the multidisciplinary team, and effective communication emerged as facilitating factors. The results also revealed a broad recognition of the importance of both family and healthcare teams for promoting patients’ adherence and engagement. Furthermore, engagement was shown to depend on a set of actions involving communication, shared decision-making, and acknowledgment of patients’ subjectivity and diverse ways of experiencing illness and treatment. The multidisciplinary team employed communication, openness to clarifications, family involvement, and the establishment of a trusting, light, and playful relationship with patients and relatives. In contrast to the biomedical model, understanding the variables addressed in this study may serve as a pathway for valuing patients’ subjectivity and fostering more humanized care, while also providing insights for improved planning of interventions in hemodialysis clinics.
